Registration

 

The Online NMAT Application Process is a Four-Step Registration process which includes

    Step 1: Filling out the personal details of the NMAT Application Form

    Step 2: Answering the Student Information Questionnaire

    Step 3: Printing the NMAT Information Form and;

    Step 4: Paying the Registration and Test Fees

STEP 1: Filling out the NMAT Application Form

 

1.       Go to the NMAT website and click on the Register Link under the On-line Options menu.

 

 

2.       The Online NMAT Application form shall be displayed on your screen and you shall be guided through the Four-Step Registration process.  Please remember to read the requirements and instructions before filling up the form.  Note also the Examination date indicated below the instructions.

 

 


 

3.       Enter your Family Name, First Name and Middle Name.

 

 

 

4.       Enter your Email Address.  To ensure that you have entered a valid email address, please click on the Send Test Email button and then access your email.  You should receive an email notification from NMAT Online [nmat@cem-inc.org.ph] with the subject NMAT Online Test Email.  If you do not receive the test email, confirm if you have entered your email address correctly.

 

 

5.       Enter your Password with a minimum of 6 characters and type it again in the Confirm Password field.  Please do not use special characters (e.g.  #,* &, etc) when encoding your password

 

 

6.       Select your Security Question from the drop-down list and type your corresponding Answer. The Security Question will be prompted when you forget your password.  We highly recommend that you select a question and answer that you will most likely remember.

 

 

7.       Review the information you have entered and click on the Continue button to continue filling up the NMAT form.  If you wish to cancel your registration, click on the Cancel button.

 

 

8.       Upon clicking Continue, your name, email address and Examination Date will appear in the next page.  Supply the succeeding information starting with the Testing Center.  You may select your choice by clicking on the drop-down list.  Note that all fields marked with the red asterisk (*) are required fields. 

 

 

9.       If you have taken the NMAT before, indicate how many times you have taken the NMAT Exam by selecting the number from the drop-down list

10.   Enter your complete Mailing Address

 

 

11.   Enter your Telephone number in the format country code, area code and then phone number.   Sample format is shown beside the box for your reference

 

 

12.   Select your Zip code from the drop-down list.  The Zip code listing is arranged alphabetically by city.  Note also that the format of the list is Town, City.  You may also click on the Search button to narrow your search to a specific province/ city

 

 

13.   Enter your complete Home Address and then select the corresponding Region/ Province from the drop-down list.

 

 

14.   Enter your Nationality and Religion.

 

 

15.   Select your Sex and Civil Status from the drop-down list

 

16.   Enter your Birth date in the format MM/DD/YYYY and then enter your Age.

 

17.   Review the information you have entered and click on the Continue button to continue filling up the NMAT form.  If you wish to cancel your registration, click on the Back button.

 

 

 

 

 

 

 

 

 

18.   Upon clicking the Continue button, you will be prompted to enter your Educational background details.  Enter the name of the College or University you graduated or will be graduating from and the corresponding College Address.

 

 

19.   Select the type of College [Public/ Private Sectarian/ Private Non-Sectarian] and then type the year you graduated or will be graduating.

 

 

20.   Select the Course you finished or will be finishing from the drop-down list.  If your Course is not in the list, select the first option [Not on the list, I will specify on the space provided for] and then specify your course in the space provided.

21.   Select your Top 3 Preferred Medical Schools.  Please select 3 different Medical Schools

22.   For repeaters, enter your previous Examination date by clicking on the Space provided.  This will show the calendar so you can select the date.  You may also type the date directly in the format MM/DD/YYYY.

 

 

 

23.   Enter your corresponding GPS in the space provided

 

 

24.   If you have taken any NMAT review courses, specify the Name of the Review Center and the corresponding Review Center Address.

 

 

25.   Indicate whether you are willing to release information on your results and mailing address to any inquiring school or scholarship agency by clicking on the YES or NO radio button.

 

26.   Review the information you have entered and click on the Continue button to continue filling up the NMAT form.  If you wish to go back to the previous screens, click on the Back button.

 

 

 

 

27.   This shall be last page for STEP 1 of the Registration process.  Review all the information you have entered and click on the button All Information Entered are Correct to proceed to STEP 2.  If you wish to change any of the information, click the button Go Back to Change Information Entered.

 

28.   Upon clicking the button All Information are correct, you will be informed that you have successfully completed Step 1.  Read the instruction carefully and then click on OK to proceed to Step 2.  If you wish to go back to the previous screen, click on the Cancel button.

 

 

 

 

STEP 2: Completing the Student Information Questionnaire

 

1.       The Student Info Questionnaire consists of twenty questions, which as mentioned by the Information Message Prompt, is a set of questions to gather personal/ descriptive information about the examinee.  Read the questions carefully and then select your answer by clicking on the box of the letter beside your answer.  The letter of your choice will be entered automatically in the ‘Answer’ box.

 

 

2.       If your answer is not in any of the choices, click on letter corresponding to the choice ‘Others (Specify)’ and then type your answer in the space provided.  Ensure that the corresponding letter for the option Others is indicated in the ‘Answer’ box.

 

 

3.       Click on Next to proceed to the next question.

 

 

4.    Answer the questions sequentially by repeating steps 1.2.1 to 1.2.3 until you’ve completed all twenty questions.  If you wish to review or go back to the previous questions, click on the question/ page number or select the page number in the “Go To” drop-down list.  You may also click on the Previous button to go back to the preceding page.

 

 

5.       On the last question, you will be asked to certify if all the information you have entered is true and correct.  Click on YES to proceed to STEP 3 or click on the button NO or Cancel to go back and edit the answers.

 

 

6.       Upon clicking YES, you will be informed that you have successfully completed the second step.  Read the instructions carefully and click on OK if you’re ready to print the NMAT Identification form. 

 

 

 

 

 

 

 

 

 

 

 

 

 

STEP 3: Printing/ Downloading the NMAT Information form

 

1.       Upon completing Step 2, you are now ready to Print/ Download the NMAT form.  Check the Print Preview by scrolling downwards or to the right and note your NMAT Application number as well as the CEM Bank Reference Number. 

 

 

2.       Click on the button Print/ Download NMAT Identification form to print or download the file.  Another browser will open to display the bar-coded printable/ downloadable NMAT form.  You may also click on the Back button to go back to the Student Info Questionnaire.

 

 

 

 

 

 

 

 

 

 

3.       To print the form, click on the printer icon or go to File > Print at the upper left portion of the screen. 

 

 

 

 

4.       To download the form, click on the Save icon or go to File > Save As at the upper left portion of the screen.

STEP 4: Paying the Registration and Test Fees

 

1.       Step 4 contains the instruction on how to pay your registration fees and all the necessary reminders prior taking the examination.  Take time to read the instructions before clicking the Done button to complete and end your registration process.  You may also click on View Instructions on the day of the Test or View BPI Payment Procedure to read further details.

 

 

2.       Click on View Instructions on the day of the Test and then click on Open to view the instructions in PDF format or click on Save to save the file in your computer for later use.  Please note that you should have a PDF/ Adobe reader installed to view these files.

 

 

 

 

3.       Read the instructions carefully and close the file/ browser once done.

 

 

4.          To view payment instructions, click on View BPI Payment Procedure and then click on Open to view the instructions in PDF format or click on Save to save the file in your computer for later use.

 

 

 

 

 

 

 

 

 

 

Editing Previous Registration

 

1.       To edit previous registration, go to the NMAT website and click on the Edit Registration Link under the On-line Options menu.

 

 

2.       Enter your email address and password and then click on Continue.  You may also enter your NMAT Application number and password and then click on Continue.

3.       Note that you can only edit your application/ registration details including the answers to the Student Info Questionnaire if you have not yet paid the registration and test fees.  If you are already paid, system will prompt that you can no longer edit your registration details.

 

 

4.       If you have entered your email address/ NMAT application number correctly, your personal information will be displayed after you have clicked on the Continue button.  You may edit any of the information you have entered during the registration process.  Once done, click on the Continue button.

 

 

 

 

 

 

 

 

 

 

 

5.       Your educational background details will now be displayed.  You may edit the information and if the details are correct, click on Continue.  If you wish to go back to the previous page, click on Cancel.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

6.       Review all the information you have entered and click on the button All Information Entered are Correct to proceed with editing the answers in the Student Info Questionnaire.  If you wish to change any of the information, click the button Go Back to Change Information Entered.

 

 

7.       Once done with editing the personal and educational background details, you may also review the answers to the Student Info Questionnaire.  You may change your answer simply by clicking on the letter of the box beside your answer.  Your new choice will be entered automatically in the ‘Answer’ box.

 

 

 

 

 

 

8.       Click on the Next button to continue editing your answers to rest of the questions.  You may also browse through the other questions by clicking on the page/ question number or by selecting the page/question number in the “Go To” drop-down list.  You may also click on the Previous button to go back to the preceding page.

 

 

9.       Same with the registration process, on the last question, you will be asked to certify if all the information you have entered are true and correct.  Click on YES to proceed to STEP 3 or click on the button NO or Cancel to go back and edit the answers.

 

 

10.   Same also with the registration process, upon clicking YES, you will be informed that you have successfully edited/ completed the second step.  You may read the instructions again and click on OK if you’re ready to print the revised NMAT Identification form. 

 

 

 

 

 

 

 

 

 

 

 

11.   Upon completing the revisions in Step 2 (Student Info Questionnaire), you are now ready to Print/ Download the revised NMAT form.  Check the Print Preview by scrolling downwards or to the right.

 

 

12.   If the information is correct, click Print/ Download NMAT Identification Form.  Another browser will open to display the bar-coded printable/ downloadable NMAT form.  You may also click on the Back button to go back to the Student Info Questionnaire.

 

 

 

 

 

 

 

 

 

 

 

 

 

13.   To print the revised form, click on the printer icon or go to File – Print at the upper left portion of the screen. 

 

 

14.   If you choose to save the file, click on the Save icon or go to File – Save As at the upper left portion of the screen.  System will ask you if you want to replace the original file.  Click on Yes to overwrite the previous file you have downloaded.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

Viewing the Status

 

1.       To view the status of existing registrations, click on the View Status link.

 

 

 

2.       Enter your email address and password and then click on Continue.  You may also enter your NMAT Application number and password and then click on Continue.

3.       Your NMAT Application Status will be displayed. 

 

 

4.       If your Application Status shows that you have already paid the registration and test fees, you may already download the Practice test and supplementary materials.  To download, click on the Download button corresponding to the material you want to download. 

 

 

 

 

 

 

 

 

 

 

5.       Click on Open to view the file in PDF format or Save to save the practice test in your computer.

 

 

6.       Close the practice test once done

 

 

 

 

 

 

 

 

 

 

 

Viewing Examination Results

 

1.       To view Examination results, click on the View Result link.

 

 

 

2.    Enter your email address and password and then click on Continue.  You may also enter your NMAT Application number and password and then click on Continue.
